The review

Look back at the last visit.

The clinician asks about changes since you last met, including your experience during ordinary activities and your response to any agreed care. A change in symptoms or a new concern belongs in that conversation before proceeding.

Your account matters alongside the assessment. An activity that feels easier, a concern that remains, or an approach you were uncomfortable with can all change the discussion.

Next steps

Continue, reconsider or involve someone else.

Further care should have a stated purpose. The clinician may suggest continuing the plan, changing it, leaving more time to review, or involving another health professional. There is no fixed number of visits that suits everyone.

Prepare a follow-up request
